Procedures For New Patients: Day One To prevent a delay in processing you as a patient, please bring an insurance card / form So we can verify your coverage. We request you complete the health history questionnaire below. Please fill in all areas, Including who referred you to our office and bring it with you to your first visit. Tubbs Chiropractic will evaluate your medical history and recommend an examination to determine if care in our office may be of benefit to you. X-rays may be necessary to determine the cause of your health problems. Day Two - The doctor will review your x-rays with you and discuss a treatment plan.
